If you are interested in being a Disaster Volunteer, you must also become a member of our National DSHR (Disaster Services Human Resource) System. Please also complete the documents below in addition to the above forms:

• DSHR Enrollment Application
  
• Personal Statement of Understanding
  
• Applicant Data Record
  
• Health Status Record
